Here's what to expect and what to have ready.
- 📋Fill out the form below completely — the more detail you share about your dog, the better we can prepare for your visit and serve you well from day one.
- 📅Once submitted, we'll review your information and reach out within one business day to schedule your consultation visit.
- 📧Once you receive a confirmation email from [email protected], reply with your dog's vaccination records — including a negative fecal test — and a copy of your current municipal dog license.
- 💳A non-refundable $50 consultation fee is required to confirm your visit. We'll walk you through payment after you submit.
What to have on hand
✓Current vaccination records — Rabies, DHPP, Bordetella, Leptospirosis
✓Negative fecal test with Giardia screening (required every 6 months)
✓Copy of your dog's current municipal license
✓Emergency contact details (someone other than primary/secondary)
✓Your vet's name and phone number
Reminder: Per Ollie's Law, all dogs are required to have their dog license on them at daycare and boarding facilities. Please make sure yours is current before your visit.
